Once he or she has signed this application, the Parent or Legal Guardian should submit the date at the time of signing. Therefore, once this application is confirmed as correct, the Child’s Parent or Legal Guardian should sign his or her name to prove that the “Oath” displayed above is true. The signature of the Child’s Parent or Legal Guardian must be submitted before a Notary Public.
PREMISSION TO USE NOTARIZED LETTER TEMPLATE CODE
Submit the telephone number where the Child’s Parent or Legal Guardian may be reached by supplying the area code and number where requested then continue with a record of the Parent or Legal Guardian’s “Email Address.” Notice that each address item will have a unique area of presentation. The physical “Street Address” of the Parent or Legal Guardian’s home address along with its apartment number (if applicable), “City,” “State,” and “Zip Code” should be produced across the spaces Section (5) provides.
PREMISSION TO USE NOTARIZED LETTER TEMPLATE FULL
The full name of the Child’s Parent (or Legal Guardian) whose consent is required to approve of this application’s submission must be presented in print on the first blank line of the consent statement provided. If the Child is under sixteen years old then the full name of the Adult who will accompany the Child to the passport office or otherwise effecting this application should be listed in Section (4B). If the Child is over sixteen years old (but still an underage Minor), place a mark in the “No” checkbox. If the Child is under sixteen years old, the “Yes” box should be selected from Section 4(A). Select the third checkbox if the goal of this application will be only to obtain a passport card for the Child. If this application is being submitted to only gain a passport book for the Child then the second checkbox must be marked.
If the purpose of this application is to obtain a “Passport Book And Card” for the Child named above, then place a mark in the first checkbox of Section (3). Select Item 3 Or Select Item 4 Or Select Item 5 Document the two-digit month, calendar date of the month, and four-digit year of the Child’s birth date in the space provided by Section (2). The name of the Child this application concerns should be displayed across the three boxes on the top row by supplying his or her “Last” name,” “First” name, and “Middle” name where requested. IRS Form DS-82 – Use as an application to renew an adult passport. This identification must be photocopied (front and back) and submitted along with the parental consent form.
The non-applying parent/guardian must sign Form 3053 in front of a notary and present a piece of government-issued photo identification.
If one of them isn’t able to appear for the application process, the non-applying guardian must grant permission by completing a statement of consent and submitting it with the child’s DS-11 Form. Both parents/guardians must authorize the issuance of a minor’s passport and must provide the proper documentation of citizenship and guardianship.
A passport parental consent, or statement of consent (Form DS-3053), is used in the event a minor (person under 16 years of age) is applying for a passport and only one of the parents/guardians can be present.
